VA Small Group Request Form

Small groups will consist of up to eight VA students and a librarian. Groups will meet at the beginning of and then periodically throughout the semester. The first meeting will be scheduled using this request form, and all subsequent meetings will be at the discretion of the small group.

Location: Bibliographic Instruction Classroom 827, 2nd floor

Form Submission:

  1. E-mail this form by clicking the "Submit via Email" button at the bottom of the page OR
  2. Print and submit in person to the Library Administrative Assistant (992-7061, opt. 3) or the staff at the Help or Reference Desks OR
  3. Print and submit via internal mail to the Library Administrative Assistant

Submissions must be received at least 7 working days  prior to the desired date of the instruction session. Please allow 48 hours for a response.
